Surveys Time is running out, they will be back and they are looking for more. We can help you thru this process.
We bring a team of experts to your facility that is an independent survey team and has the necessary objectivity, the ability to provide adequately developed plans of correction and an realistic expectation of what will happen with the future State Survey visit. Our survey team can include a Licensed Physician, Registered Nurse, Social Worker, Administrator, Dietary Coordinator, and a Licensed Psychologist. Each individual is selected because of their expertise and service to Long Term Care Facilities. We provide an objective and critical assessment of the performance of each area in your facility. We survey and report with the State Surveyor team in mind--- critiquing and instructing as we go. We discuss our findings with each Department Head individually and point out possible plans of corrections and/or solutions to the problems we have identified. Just as with the State Survey, we provide you with a complete exit interview with the Administrator and Department Heads. You will receive a confidential detailed report of problems and/or deficiencies, including identifying appropriate tags and levels of severity one may expect from the State Survey Team should the problems we identify at your facility fail to be corrected. Our survey process can be tailored to your requirement and budget. We can begin with a simple spot check in one area if you have any specific areas of concern. The survey we suggest takes about two days, but may require as many as three or four days, depending upon the size of the facility and the problems encountered. Additionally, many of our surveyors are educated by the JCAHO, and have experience with all current JCAHO standards and interpretation as JCAHO surveyors. They have the expertise to and will evaluate the following process issues: (1) Assess the patients rights function (2) Review the implementation and documentation of patient assessment function (3) Evaluate the mechanisms for the provision of care (4) Assess the patient and family education function (5) Evaluate the continuum of care function (6) Evaluate the performance improvement function (7) Evaluate the leadership function (8) Perform a brief facility site walk-through to assess life safety code issues and the environment of care function (9) Assess the Human Resource Management function (10) Evaluate the Information Management function (11) Evaluate the processes used for surveillance, prevention, and control of infection (12) Evaluate the Medical Staff structure (13) Assess the governance, management, and nursing leadership structure (14) Educate the medical and clinical support staff relative to improving organizational performance standards (15) Assist in the development of strategic plans for improving organizational wide functions (16) Prioritize recommendations based on the organization's environment (17) Develop work plans. There will also be a one day focus on provisions of Ambulatory Care Service, and one day for Medical Records review and contracted services.
